Supportive Living/In-Home Family Based Supports

Threshold Rehabilitation Services Inc. provides supportive living services, which are for individuals who live independently within their own apartments or homes.  Threshold staff members meet with individuals in their own home or a community location to provide the service.

Services are customized based on individuals needs and preferences and can include:

• Assistance with medical appointments
• Budgeting and money management
• Grocery shopping assistance
• Food preparation instruction
• Public transportation training
• Recreational activities
• Use of community resources
