Community Hospital Integration Project Program (CHIPP)


Threshold provides assistance for individuals' transitioning from State Hospitals into the community. All referrals for the CHIP Program are selected by Service Access & Management.

The CHIP Program provides a full complement of clinical support which includes a psychiatrist, a psychiatric nurse, medical assistant, and program coordinator.

The program provides residential supports in the community in a variety of settings including a group home, family living home, independent apartment with supports and personal care boarding homes. Threshold's CHIPP services are delivered following recovery principles.

The recovery philosophy ensures that individuals have full participation and choice in all services delivered including support staff and promotes dignity and personal achievement in the community.
