Threshold provides assistance for individuals transitioning from state hospitals
to the community. The Community Hospital Integration Projects Program (CHIPP) provides
a full complement of clinical support which includes a psychiatrist, a psychiatric
nurse and program coordinators. A key component of CHIPP is supported living. Supported
Living Specialists assist with budgeting, grocery shopping, transportation, and
daily living skills. For those individuals who need a more intensive level of care,
family living services are available. Through this service, the individual lives
full time with a family within the community. For those people who are hospitalized
in an acute inpatient unit diversion and respite services are provided to avoid
a state hospitalization.
